What Host Home Insurance does not Cover

Home means something different to everyone and some of our host homes have families that the person can live with, other host homes may be just two people living together. We support host homes with 1-2 people receiving services living in the home with a designated Host Home Provider. On some occasions, if it is the persons' choice to live with three people receiving services, Overture and the Case Management agency may work with that host home to get a three person waiver to allow for this living arrangement. • AbleLight’s shared living arrangement, also known as a Host Home, provides a family like support environment for people with intellectual and developmental disabilities.
Additionally all potential Host Home Providers are required to participate in Laradon’s Host Home Provider orientation. During the orientation, individuals will learn about the various policies and procedures regarding residential documentation, monitoring, quality standards and other related topics. You see, our mission is much more than simply providing cost-effective care and innovative services. It’s about the less tangible, but far more important – enabling the means for a fullness of life that each one of us deserves. Our goal, through our matching process, is to ensure that you and the person who comes to live with you know each other well and will be able to live together comfortably. As a Host Home Provider, you can earn a stipend between $1,800 – $4,000 per month.

The rates are based on what we receive from the state of Colorado, based on each individual person’s Supports Intensity Scale . The SIS assesses their level of support need, which determines reimbursement rates. Every person is different so please contact Intake with specific questions. Provide coverage for the personal property of the Host Home Provider or the consumer.
We may have a meeting or receive a call and move the person the same day/week. We could also have had meetings earlier on and then receive a call out of the blue for an urgent move. The person may not have all of their belongings and will very likely need a bed already available in their room. You may not have all the information you would like to have about the person in services, especially if this is a new/external placement or emergency enrollment. In this instance, Overture will partner with you to learn more, and will share new information as it becomes available.

**Host Home Insurance requirements in the State of Colorado**
Members must meet financial and medical eligibility, as well as program-specific criteria to access services under a waiver. The applicant must be at risk of transition to a nursing facility, hospital, or Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). To utilize waiver benefits, members must be willing to receive services in their homes or communities.

Providers enrolling with an EIN must submit a statement on letterhead attesting that the provider is the only person who will be providing services and that he/she will not be hiring others to provide these services. If the provider will have employees providing services, the provider must submit a list of staff and attach a copy of the current license or certification and resume for each employee. Providers enrolling with an SSN do not need to provide an attestation. Providers enrolling with an EIN must submit a statement on letterhead attesting that the provider will be the only person who will be providing services, and that he/she will not be hiring others to provide these services.
A computer is required to utilize a web-based electronic records system. Host Homes are residential settings in which an individual receiving Overture services lives in the community with someone of their choosing that is available to help provide supports when needed. Host homes come in all shapes in sizes depending on what the person is looking for in a living companion. Overture has host homes throughout the state of Colorado in various cities, suburbs, and more rural areas.
Home means something different to everyone and some of our host homes have families that the person can live with, other host homes may be just two people living together. We support host homes with 1-2 people receiving services living in the home with a designated Host Home Provider. On some occasions, if it is the persons' choice to live with three people receiving services, Overture and the Case Management agency may work with that host home to get a three person waiver to allow for this living arrangement. • AbleLight’s shared living arrangement, also known as a Host Home, provides a family like support environment for people with intellectual and developmental disabilities.
Children's Extensive Support (CES)

Host Home Providers must be 21 or older, pass a background check, obtain Host Home insurance, homeowner or renter’s insurance, automobile insurance and a business trade name with the State of Colorado.
We work to help individuals in services develop natural supports and friendships within their community. Our agency provides opportunities for greater choices to increase an individuals control over their own life. Host Home Providers are responsible for basic home care and related responsibilities. This includes providing nutritious meals, transportation to medical appointments and recreational activities, ensuring access to community activities, and supervision based on the needs of the individual you are supporting.
Employment Services
The IHSS agency may only serve contiguous counties disclosed on their most recent home care license renewal application. Out of State providers must attach a signed attestation, on letterhead, that states the provider is enrolled in their own state's Medicaid Program as a meals provider. Must submit applicable licensure for Program Director to include Licensed Psychologist with demonstrated applicable experience, or LCSW, PT, OT, SLP, RN or Masters level Psychologist with demonstrated applicable experience. Must submit a copy of the license along with a resume, transcript and/or curriculum vitae for review. If you are planning to include this service in your application and plan to enroll using your SSN rather than an EIN, please do not pay the application fee . One year experience working with individuals with paralysis or other long-term physical disabilities.
