Because we like to do things differently – every client, every time. Which is why we’ve invested in creating unique environments that don’t just accommodate people, they craft a culture that facilitates growth, offers flexibility and engenders fulfillment. Host Home Providers must be 21 or older, pass a background check, obtain Host Home insurance, homeowner or renter’s insurance, automobile insurance and a business trade name with the State of Colorado.
If the provider will have employees providing services, the provider must submit a list of staff and attach a copy of the current certification, degree, and resume for each employee. For each individual providing services, providers must submit a license and a resume showing individual/family grief, loss, or bereavement counseling with pediatric/adolescent experience of one year. It is our philosophy that choice, experience, and participation are fundamental to the personal growth and fulfillment of people in our care. We believe that people with developmental disabilities are part of the community, not simply recipients of services.
Become a Host Home Provider
Some clients have significant medical or behavioral challenges. Some may need assistance with mobility, bathing, or toileting. Some clients are very independent and may have few needs.

When using a combination of experience and education to qualify, the education must have a strong emphasis in a human behavioral science field. It is all about finding the right fit for the person seeking residential services, as well as for you and your individual home. Timelines may vary from a few weeks to a few months or longer.

The rates are based on what we receive from the state of Colorado, based on each individual person’s Supports Intensity Scale . The SIS assesses their level of support need, which determines reimbursement rates. Every person is different so please contact Intake with specific questions. Provide coverage for the personal property of the Host Home Provider or the consumer.
We may have a meeting or receive a call and move the person the same day/week. We could also have had meetings earlier on and then receive a call out of the blue for an urgent move. The person may not have all of their belongings and will very likely need a bed already available in their room. You may not have all the information you would like to have about the person in services, especially if this is a new/external placement or emergency enrollment. In this instance, Overture will partner with you to learn more, and will share new information as it becomes available.

How does RoundUp Fellowship support its Host Home Providers?
To ensure that all individuals are supported in this, and that all staff are trained on how to provide this support. This approach creates long-lasting relationships that lead to growth for both the person in services and the provider. Host Home Providers may work with up to two individuals with disabilities in their home, depending on the number of bedrooms/bathrooms and the amount of shared space that is available. Individuals are able to choose their roommates, and a 30-day trial period is required to ensure the Host Home is an appropriate placement for the individual if agreed by entire team.
Continuum of Colorado is a 501, nonprofit agency providing services to people with a wide range of abilities. An assortment of technologies are available to individuals in Imagine! Technologies used are chosen on the basis of how they can best support an individual in meeting their needs, goals, and desires, how they can support increasing self-reliance, and how they can support caregivers. Host Home Providers receive payment the first of each month for the service provided during the previous month. The daily contractual payment is based on an individualized payment structure established by the State of Colorado, Division of Developmental Disabilities and based on an assessment of the needs of the person receiving services.
